Reports showing location, names of teachers, principals, and assistant principals, their salaries, total amount expended for instruction, number of school days per year, institution from which the principal graduated, and notarized signatures of each board member. In some cases notarized Certificates of Establishment and Maintenance and initial requests for state aid are attached to the report.
